Zephyr Squad test steps and execution test steps/attachments have disappeared

We identified that one of our zephyr squad tests, suddenly has no test steps or other details. What's even more alarming is, the past exectution of this test is also empty and has none of the steps or attachments there anymore. 

This has happened before, where test steps randomly dissapear and sometimes appear. Has anyone else seen this issue, and if so, how did you fix?
