("iexplore", 2) means it's the second instance of the internet explorer process. Probably running in the background without a window, just need to kill all IE processes.

Thanks for the response. I have checked and there is only one instance of IE running. Any other guesses ?

Hi HealthAutomate,
Like cunderw says it's another IE process running in your system.
Check whether the multiple processes of IE are disabled like it described here >>

If above doesn't help, then I was able to resolve this by editing Name mapping Index value to 2 which fixed the problem.
