Here are the usual suspects:
There could be numerous other factors, and at a point it becomes simpler to just assume there will always be more causes of divergence than you can control.
What I found was most sensible was to build code around image comparisons so that more than one reference image can be kept on file as a backup to more elegant methods to ensure consistency.
Screen resolution
Color depth settings
Display DPI setting
Operating system
Status of ClearType text setting
Graphics drivers
