Hi bhakti,

 

It sounds like you need to modify the Issue-Tracking Templates to specify the fields you want to see in this dialog. Please read the "Using Issue-Tracking Templates" ( http://smartbear.com/viewarticle/56311/ ) help topic to learn more about this.
