I heard back from SmartBear Customer care today.
The key was to make sure you have checked "*" from MSAA options
- Goto Tools > "Current Project Properties"
- Select "Open Applications" > "MSAA"
- Add a checkmark the the "*" entry at the top, which is unchecked by default
