As far as I can see, this is how Internet Explorer works. Perhaps, Microsoft did this due to certain security reasons, I don't know...
According to your results, the VisibleOnScreen property changes after you set the focus to the button. Did you try to set a delay after the VisibleOnScreen property becomes true, and click the button after the delay? Maybe setting the focus is the thing that "unblocks" the button. (The issue is not reproduced on my computer, so my post is full of suggestions, sorry :-) ).
As for the keyboard input, the default button in the dialog is "Cancel", that is why, it gets the ENTER keystroke.
