Anyone else experiencing a high number of Invalid Credentials errors when logging into QAC (11.0)?

We're on On-Premise customer on QAComplete 11.0 using IE11 and Active Directory to authenticate.  We seem to be experiencing a high volume of Invalid Credentials errors when attempting to log into QAComplete.  Just curious if others are experiencing this behavior?
