Questions about Code Review process

Hi,



I'm new to Smart Bear Code Review and have some questions about the process:

1) Does the Code Review approval is on file level or Change-set level?

2) When Code Review decide to approve, does it closes the file to changes? (like change-set complete action?)

3) If The developer wants to edit the file after the Code Review approval, is it possible?

4) Code Review grade for a single file (with history)?
