I have created a bunch of test cases, cycles and plans and have then created Confluence pages to document them through.
When i save, i get the "Traceability" button at the top of my screen, my colleague does the same and he gets no traceability button.
Is there a setting somewhere?

AFAIK there are no macros in use unless our admin team have employed some.

Can you confirm that your colleague has access to the test cases, cycles, and plans you have linked to that Confluence page?
If he has access, this might be a bug related to the plugin. In this case, I will ask you to please file a support ticket at Server Support Portal

My colleague does have access to the cases/cycles/plans and he has the traceability links within those pages and can see the created confluence page too, Its just the traceability "button/icon" on the confluence page that is not present
I have tested with another colleague and he does have the button, so seems user related and not app.
I see, @Hightech. Do you know if your colleague has special characters in his username?
No special characters that i can see unless they are hidden ones
