Windows open file dialog not detected when recording test despite having been mapped by testcomplete

Hi all, 

I'm recording tests and part of the test is to upload a file. This uses a windows open file dialog which TestComplete has mapped previously as  "dlgFileUpload". 

However in a recent recording the open file dialog is not being detected as the mapped item and is instead listed as "panel3" in the keyword test window.

All previous test recordings have been able to detect the open file dialog mapping, as dlgfileupload, this is the first test where I have had this issue. 

Any ideas as to why the mapping is not being detected??

Things to consider:

If you map the dialog box manually, which name do you get?

Does your name mapping by any chance have a duplicate entry for that dialog box?  

Did you upload a Windows patch recently?

Are you working on the same machine and the same version of Windows as with the previous tests?

Has your app under test changed?
