One of the KPIs for QA teams' success is Defect Leakage (i.e. What percentage of defects reach production envs vs. are discovered in lower envs). I'm surprise this isn't already a feature of Zephyr to be honest. We would need some field to specify environment while creating defects in Zephyr, and an associated dashboard or report for QA managers to review.
