TestComplete Quick Checkpoints not working correctly

This sounds like Chrome 99.04 is not supported by TC 15.40...

Assuming that, as you are saying, all required documented setup has been done, I would create Support ticket via the https://support.smartbear.com/testcomplete/message/ form.
