Points 1 and 2 can be done with the help of AQtime profiler, another tool by SmartBear (http://smartbear.com/products/qa-tools/application-performance-profiling/).
As for the point 3, I am doubting that this is possible at all unless your application provides some metadata (like .Net does) or debug information (like debug builds do).
