unable to add files to review

I am getting this error when I attempt to add files to a review. I can create the review OK, its just the adding files that isn't working.



"An error occurred trying to Create new Review" - the 'reason for the error' is just some html output.....



See attached error.



I am getting this same error when adding either perforce changelists or local files via code collaborator gui (v7.0.7024) or eclipse (code collaborator plugin v7.0.7024).



Other members of my team can upload files the the same review OK. Code collaborator server is v7.x



Any help would be much appreciated.
