If a Cascade custom field is mandatory, all existing levels should have a value

When you create a custom field of type Cascade and set it to mandatory, the validation only checks that the top level (parent item) is filled out. It should also make sure that the second level (sub-item) is filled out if any exist for the selected top-level value.
