From my understanding, the MCP agent itself doesn't establish a VPN tunnel to your application. It acts as the bridge between the AI service and your local environment, so as long as the agent can reach your on-prem application and the required outbound communication is allowed, testing should be possible. The exact setup probably depends on which SmartBear product you're using (ReadyAPI, TestComplete, etc.), so it would be helpful if SmartBear could clarify the network requirements and whether any inbound connections are needed.
