[CodeReviewer] Failure to connect to CodeCollaborator server

When attempting to add a shelve set for code review I'm getting a message "Could not connect to a Collaborator Server at http://<hostname>/xmlrpc/secure: Does not appear to be a running Collaborator Server".  

Re-installed the app again and am still seeing the same alert.  I have verified the URL is not a valid page, but want to know how does the string "xmlrpc/secure" get appended to the host name since I did not have it specified in my setting?
