Limiting commits to a single review using ensure-reviewed

I am using a pre-commit hook with ensure-reviewed. It works fine however nothing prevents a user from making commits using a previously used review. Is it possible that a review could be used only for 1 commit and force future commits to use another review id?

While not as important, is it possible to exclude a folder from requiring a review id? ie we have a Test folder in our SVN repository. Changes in this folder don't need to be reviewed.
